If you would like to make a reservation for a session at our clinic, we are accepting request reservations.
Please fill in the required items and send us the form.

Please be sure to read the “Privacy Policy” when using the request reservation form.
We accept request reservations 24 hours a day, but we can only reply within the business hours.
We try to provide a prompt response, but we can only reply within the business hours.
All responses will be done by email.
